If you have seen Dr. Locknane in clinic at his previous setting we would still appreciate you taking the time to fill out the forms above. This will allow Locknane Athletic Medicine to build a complete patient folder.

Past medical files at any other location may be requested by you at your previous clinic and brought in by you. Dr. Locknane does not require you to bring previous files and believes a brief discussion of your medical history will most often be sufficient.
